1. Primary application documents
These are the standard forms required for all housing loan applicants:
Housing Loan Application Form (HLAF): Two (2) copies, duly accomplished with your 1x1 ID photo.
One (1) Valid Government ID: A photocopy (back-to-back) of any major ID (e.g., PhilID, Passport, Driver’s License, or PRC ID).
Virtual Pag-IBIG Selfie: If applying online, a clear photo of you holding your valid ID is required.
2. Proof of income (The Self-Employed Suite)

For Business Owners (Sole Proprietorship/Partnership):
Income Tax Return (ITR): A certified true copy of BIR Form 1701 for the year immediately preceding the application.
Audited Financial Statements (AFS): Stamped received by the BIR, typically for the last two years to show business growth.
Official Receipts (ORs): Proof of tax payments from the bank.
Business Legitimacy Docs: Photocopies of your DTI Registration and your current Mayor’s/Business Permit.
For Freelancers & Commission-Based Professionals:
Commission Vouchers: Reflecting the issuer’s name and contact details for the last 12 months.
Bank Statements/Passbook: The last 12 months of transactions (highly recommended for freelancers to show regular deposits).
Certificate of Engagement: A document (HQP-HLF-111) issued by your client or the business owner availing of your services.
For Rental Income Earners:
Lease Contracts: Photocopy of valid and notarized rental agreements.
Tax Declaration: Proof of ownership for the property being rented out.

Whether you are buying from a developer or a private seller, these documents are essential for the appraisal:
Transfer Certificate of Title (TCT/CCT): A photocopy of the latest title in the name of the seller.
